Fill out whatever information you wish. Minimal requirements are a first name and daytime contact phone. If you are requesting in-home service we will require the model # of the TV before we come out. Indicate times that are available for the service call to expedite scheduling. 

If the set is under warranty or extended warranty we will eventually need the date of purchase, serial # and place of purchase. If this information is not available now we can get it later.
